I confirm that I consent to participate in hypnotherapy or counselling sessions provided by Karen Boxall, hereafter referred to as ‘the programme’. I agree to be an active participant and share responsibility for the progress and results of the programme. I agree to inform Karen of any changes in my circumstances or medical status that may affect my ability to participate fully in the programme. I understand that there are no guarantees as to the results or outcomes of the programme as everyone is different and I am ultimately responsible for my own decisions, actions, choices and emotions during and after participating in the programme and that I have the right to refuse to participate in any process at any time. As a service, the hypnosis sessions may be recorded. I agree that any recording is for personal use only and should not be listened to whilst driving, operating machinery or whilst caring for another person.
Payment is due upon booking the appointment, and, for continuing sessions, at the preceding appointment, thank you
